After Hours Action Plan in RACHs codesign and networking workshop (Mackay)

For RACH managers, GPs, nurses, QAS, emergency department, HHS, and in-reach staff
With funding available from the Department of Health and Aged Care, Northern Queensland Primary Health Network (NQPHN) is seeking to consult with frontline workers on how out-of-hours supports for residential aged care homes (RACHs), residents, and the workforce can be enhanced to optimise outcomes.
NQPHN and the Residential Aged Care Facility Support Service (RaSS) invite RACH managers, general practitioners, nurses, Queensland Ambulance Service, emergency department, Hospital and Health Service (HHS), and in-reach staff to join us at one of three human-centred codesign breakfast workshops.
The 90-minute codesign workshop will comprise of interactive activities, discussions, and reflections.
Learning outcomes:
- referral pathways
- the RaSS aged care pathways
- services mapping exercise and resident journey
- what works and what doesn’t
- shared action planning.
