Northern Queensland Primary Health Network (NQPHN) would like to better support older and palliating people with easier access to primary healthcare providers.
NQPHN invites Mackay residents 65 years and older and their families or carers to a free lunchtime forum to learn about the work we are doing to support you.
We also invite you to share your stories around healthcare. Your feedback will inform us and the work we do with general practitioners and other primary care providers to improve your healthcare journey.
What we will talk about:
- who we are and what we do
- what we are planning for older people, dementia, and palliative care
- if you live in an aged care home or support family:
- how telehealth can help you to connect with providers
- how we can help aged care homes to help you stay well after-hours.
What we want to hear about from you:
- how you feel about aging at home when you aren't well
- how you learn about palliative care services where you live
- how you learn about dementia and services where you live.
