Participation in this research is voluntary. If you choose to participate you would be enrolled in the CHAT-GP program and will gain access to the new CHAT-GP PEN CS Topbar application that links to your practice software (Best Practice or Medical Director). This aims to reduce the time required to do a Heart Health Check and make it easier to communicate risk and options to your patients.
